Thank you for choosing Lexington-Bedford Veterinary Hospital to care for your pet. Downloading and filling out the appropriate forms prior to your appointment will greatly assist us in adding you and your pet to our system. You may provide by:

*  If you are an established client bringing in a pet who has not yet been seen at our hospital, please fill out the appropriate species information form only.  New Clients are asked to fill out the New Client Form, as well as the appropriate new patient form above, prior to your first appointment will greatly assist us in adding you and your pet to our system.
